Your answers will be one of: text, numbers or selecting one of the "option" buttons provided. Note that on the "preparative document" (on the regional website for download) you have to put your cursor over the answer tab to see the pop up options). Your are strongly urged to create a back-up by printing and/or saving the form on line when done as it has been known, very rarely, for the system to suffer data loss!
